Demi Moore's daughters, Rumer, Scout and Tallulah, are growing increasingly worried over the fragile state their mother is in! Not only is Demi reportedly acting erratic, but the girls fear she needs serious help ' again!

'Rumer, Scout and Tallulah are so concerned about their mom that they're considering if they should stage an intervention,' a source close to Demi's daughters told Radar Online.

The girls currently have a strained relationship with their mother, and while they desperately want to help her, Rumer, Scout and Tallulah aren't sure what the next move should be! 'They don't know what to do to help her, and they're starting to wonder if she needs help again.'

Strained relationships aside, the girls have their mother's best interest at heart! They want to see Demi happy and healthy again!

'The last thing these girls need is dealing with their mother going through yet another episode,' the source adds.
